How to create an application form

Why is it relevant?: This feature helps you gather information about the clients who are signing up for your services. It allows you to decide which clients can join the program you've created and benefit from your services.

Recommendation: It's a good idea to create an application form if you want to learn about your clients beforehand or if you have any specific questions you'd like to ask in advance.

This guide will walk you through the steps of how to create an application form. 

There are two ways to create an application form for a service.

1 - Using the Banner Prompt

Step 1: A banner will prompt you to create an application form after a service has been submitted and made live.

Step 2: A pop-up window will appear, allowing you to add questions and select answer types according to your requirements.


Step 3: Once you're done, click the "Save and Exit" button, and your application form will be created.

2 - Use the "Forms" section located on your Dashboard

Step 1: From your coach dashboard, click "Forms" on the left side of the page. Then click "All Forms," which will take you to a page where you can create a new application form.

Step 2: Click the "Create Application" button, and a pop-up will appear.

Step 3: Select "Create From Scratch", then click "Next" or if you have want to use an existing one choose the option "Choose from Prior Template", click "Next", and then from the drop-down select one of the existing forms you have created and click "Next".

Note: The service must be live to create the application form.

Step 4: Enter a title for your application form and click "Next".

Step 5: Add your desired questions and the answer and select the answer type that you would like your clients to use. There are three options that you can choose from: 

1- Single Choice
2- Multiple Choice
3- Written Answer

Step 6: Once you add all the questions, click "Save and Exit."
