How to Add Custom Enrollment Forms
Follow these steps to create your enrollment form:
Step 1: Go to the Enrollment Form Section
While creating a service, go to Step 5 — you’ll see an option to Add an Enrollment Form.
Step 2: Add a Question
Click Add a Question.
Step 3: Choose the Answer Type
Type your question, then choose the answer type.
If using multiple-choice options, add your choices accordingly:

Single Choice: Clients pick one option

Multiple Choice: Clients can select more than one option

Written Answer: Clients type a free-form response
Step 4: Add More Questions
Repeat the process to include as many questions as you’d like.
Step 5: Save and Continue
Once you’re done adding questions, click Save and Next.
Your enrollment form will now appear automatically for clients when they sign up for your service.
